Formedee is the data controller for the personal data described in this policy in connection with our website www.formedee.eu ("Site").
References to "Formedee" in this policy also extend to our affiliated entities as they are established, including Formedee AG (Zug, Switzerland) and Formedee Sp. z o.o. (Rzeszów, Poland), where they act as joint or separate controllers of personal data collected through the Site.

We do not knowingly collect special category data (such as health data about you personally) through the Site's contact channels. If you are a patient, please do not send us health information about yourself through the contact form or by email.
